When an auditor escalates an item it comes here. Risk level 3 (RPN 16 to 23) needs the QA head's signature. Risk level 4 (RPN 24 or more) needs the QA head first and then an executive. The meaning of each signature is recorded, and nobody signs an item they escalated or signs twice.
